Talbot Health Services provides ASAM Level 3.5 residential care -- the highest level of non-medical residential treatment available. Our program delivers 24-hour clinical staffing, structured therapeutic programming, and immersive recovery support for individuals who need more than outpatient care can offer.

Serving Toledo, Mansfield, and Clyde, Ohio -- not all treatment centers offer this level of care.

What Is ASAM Level 3.5?

ASAM Level 3.5 -- Clinically Managed High-Intensity Residential Services is a level of care defined by the American Society of Addiction Medicine (ASAM) for patients who require 24-hour structured residential treatment but do not need the acute medical monitoring of a hospital or medically managed inpatient setting (ASAM Level 4).

In practical terms, this means patients live on-site in a residential treatment facility with around-the-clock clinical staff. They participate in a highly structured daily schedule of therapeutic activities -- typically 20 or more hours of clinical programming per week -- including individual therapy, group therapy, psychoeducation, life skills training, and medication management.

This level is designed for individuals whose addiction and/or co-occurring mental health conditions are severe enough that they cannot safely or effectively engage in recovery while living at home or attending outpatient treatment. They need the immersion, structure, and safety of a residential environment -- but they do not require acute medical detoxification or hospital-level medical care.

24-Hour Clinical Staffing

Licensed clinical staff on-site around the clock. Nursing coverage for medication administration, health monitoring, and crisis response at all hours.

Structured Programming

A minimum of 20 hours of clinical programming weekly. Every day follows a defined therapeutic schedule from morning through evening.

Not All Centers Offer 3.5

Many treatment facilities only offer lower levels of residential care (3.1). ASAM 3.5 requires significantly higher staffing ratios, more clinical hours, and greater organizational capability.

Who Benefits from ASAM 3.5 Residential Care?

Placement at ASAM Level 3.5 is determined through a multidimensional assessment using the six ASAM dimensions. Patients who meet criteria in one or more of the following areas are candidates for this level of care.

Dimension 1: Withdrawal Potential

Patients who have completed acute detox (ASAM 3.7/3.7-WM) or whose withdrawal risk can be managed through clinical monitoring without intensive medical intervention.

Dimension 2: Biomedical Conditions

Medical conditions that are stable enough to be managed in a residential setting with on-site nursing but do not require 24-hour medical or nursing care at a hospital level.

Dimension 3: Emotional/Behavioral/Cognitive

Moderate to severe emotional, behavioral, or cognitive conditions that require a structured 24-hour environment to ensure safety and therapeutic engagement.

Dimension 4: Readiness to Change

Patients with high resistance to treatment, poor engagement history, or ambivalence that requires an immersive environment to build motivation and commitment to recovery.

Dimension 5: Relapse/Continued Use Potential

High relapse risk in less structured settings. The patient requires 24-hour structure and clinical support to interrupt chronic relapse patterns.

Dimension 6: Recovery Environment

Living environment is dangerous, unsupportive, or actively enabling continued substance use. The patient needs removal from their current environment to begin recovery.

Important: ASAM level of care is not determined by a single factor. Our clinical team conducts a comprehensive biopsychosocial assessment at intake to evaluate all six dimensions and determine whether ASAM 3.5 is the most appropriate placement -- or whether a different level of care would better serve the patient. This individualized approach ensures every person receives exactly the intensity of treatment they need.

ASAM 3.5 Program Components

Our residential program integrates multiple therapeutic modalities into a comprehensive, evidence-based treatment experience designed to address the full spectrum of each patient's clinical needs.

Individual Therapy

One-on-one sessions with licensed clinicians using evidence-based modalities including CBT, DBT, motivational interviewing, and trauma-focused approaches. Minimum of two individual sessions per week.

Group Therapy

Multiple daily group sessions covering relapse prevention, coping skills, emotional regulation, grief and loss, anger management, and interpersonal effectiveness.

Psychoeducation

Structured educational programming on the neuroscience of addiction, co-occurring mental health conditions, medication education, and the recovery process.

Life Skills Development

Practical skill-building in areas such as financial literacy, nutrition and wellness, communication, conflict resolution, employment readiness, and healthy relationship building.

Family Involvement

Family therapy sessions, psychoeducation for loved ones, structured visitation, and family program days designed to repair relationships and build a supportive recovery network.

Medication Management

On-site psychiatric prescribers conduct regular medication evaluations. Medication-Assisted Treatment (MAT) using Suboxone and Vivitrol is available for opioid and alcohol use disorders.

Discharge Planning

Comprehensive discharge planning begins at admission. Clinicians coordinate step-down to ASAM 3.1, IOP, outpatient services, MAT continuation, sober living, and community support.

Wellness & Recreation

Structured physical activity, mindfulness and meditation, yoga, art therapy, journaling, and peer-led recovery support activities integrated throughout the weekly schedule.

Licensed nursing staff provide 24-hour clinical monitoring, medication administration, vital sign tracking, and health assessments throughout each patient's stay.

Licensed Therapists & Counselors

LISW, LPCC, and LICDC-credentialed clinicians deliver individual therapy, group facilitation, family sessions, and specialized trauma treatment.

Peer Recovery Support Specialists

Certified peer supporters who bring lived experience to the treatment environment, offering mentorship, encouragement, and practical recovery guidance.

Case Managers & Discharge Planners

Dedicated staff coordinating insurance authorizations, community resources, housing referrals, and seamless transitions to the next level of care.

Length of Stay

30 Days

Stabilization & Foundation

Initial stabilization, treatment engagement, early skill acquisition, and assessment of response to programming. Appropriate for patients with strong external support systems.

60 Days

Deep Therapeutic Work

Extended time for trauma processing, relapse prevention skill consolidation, family repair work, and development of a comprehensive aftercare foundation. Most common recommended duration.

90 Days

Comprehensive Recovery

Full-spectrum treatment for patients with chronic relapse history, severe co-occurring disorders, limited recovery environment, or complex clinical presentations requiring sustained immersion.

Length of stay is determined by clinical need, not by a fixed calendar. Your treatment team conducts ongoing assessments and adjusts the treatment plan based on your progress across all six ASAM dimensions. Insurance authorization is managed by our utilization review team throughout your stay.
